At Happy Health Home Care we excel at helping NDIS participants through life stage transitions. There are many transition points throughout a lifetime – starting school, leaving home, getting that first job, or retiring from work, for example. You can benefit at each of these stages of life with a planned approach that examines individual abilities, needs and goals. With proper planning and networking, we can support you through life’s transitions and on to achieving your goals.
Our Life Stage Transition program includes short- and long-term support that focuses on assistance to live at home, community participation, and helping you to coordinate your own support. This includes:
We support participants to plan for change, including developing strategies to cope with the stress that change can have. Our approach is individualised for each participant, taking into account their abilities and needs. All of this is to help participants achieve their goals both in the short term and long term.
Some of the support we provide includes the preparation of schedules (daily, weekly and/or longer term). This is to help make sure that the support being provided is assisting a participant’s life stage transition goals. We can also help with the budgeting process so that expenses are planned for and paid for when due.
Our assistance with life change transition includes short and long-term supports. The focus is on strengthening the participant’s ability to coordinate their supports, and to assist them to live comfortably and participate in their community.
